All entries must be submitted on this form or at the Contest e-mail address if the form proves troublesome. No postal mail entries will be considered.

Winner will be notified by e-mail within 48 hours of contest’s closing date and time.

Please respond to prize notification within 48 hours or another name will be drawn.

Official announcement of the winner’s name will be posted on my website Contest page.

If you don’t wish your name to be posted, you must inform us of this when you are notified that you have won.
